Client health and safety waiver

1. Parties. This waiver is between Nail Salon and the client named in this booking.

2. Services and risks. Services may include acrylic sets, Gel-X, builder gel, gel polish, nail art, fills, removals, manicures and pedicures. These services use professional products such as acrylics, gels, adhesives, primers and solvents. Possible risks include skin irritation, allergic reaction (redness, swelling, itching or pain), nail weakening from improper aftercare, and sensitivity to UV/LED lamps.

3. Your confirmation. I confirm that I have no known allergies to nail products, or I have told Nail Salon about my allergies or skin conditions before my appointment. I will tell my nail technician right away if I feel pain, burning, itching or discomfort during the service. I understand I should seek medical care if irritation or swelling does not improve.

4. Liability release. I, the client named in this booking, release Nail Salon from liability for injuries or damages resulting from nail services, to the fullest extent allowed by law.

5. Consent. I have read and understood this waiver and I agree to receive nail services from Nail Salon. I confirm that the information I provided is accurate.
